ASCVD Risk Score Calculator
This calculator estimates your 10-year risk of developing atherosclerotic cardiovascular disease (ASCVD) based on the Pooled Cohort Equations from the American Heart Association/American College of Cardiology guidelines.
How to Use This Calculator
- Enter your age (between 40-79 years)
- Select your gender and race
- Input your latest cholesterol levels (total and HDL)
- Enter your current systolic blood pressure
- Indicate whether you're on blood pressure medication
- Select your diabetes and smoking status
- Click Calculate to see your 10-year ASCVD risk percentage
Formula Used
10-Year ASCVD Risk = 1 - Baseline Survival ^ (exp(Sum of β-coefficients - Mean Sum of β-coefficients))
Where:
- Baseline Survival = S0(t) value specific to gender and race
- β-coefficients = Weighted values for each risk factor
- Mean Sum of β-coefficients = Average values for the population
This calculator uses the Pooled Cohort Equations developed by the American Heart Association/American College of Cardiology (AHA/ACC) risk assessment guidelines.
Example Calculation
Real-World Scenario:
A 55-year-old African American male wants to assess his cardiovascular risk. He has his recent lab results and blood pressure readings.
Given:
- Age = 55 years
- Gender = Male
- Race = African American
- Total Cholesterol = 210 mg/dL
- HDL Cholesterol = 45 mg/dL
- Systolic Blood Pressure = 140 mmHg
- Blood Pressure Treatment = Yes
- Diabetes = No
- Smoker = Yes
Calculation:
Using the Pooled Cohort Equations for African American males, the calculator processes these risk factors to determine the 10-year risk.
Result: 15.2% 10-year ASCVD risk, which falls in the "High Risk" category, suggesting consideration of statin therapy and lifestyle modifications.

Accuracy Notice
The Pooled Cohort Equations have been validated in large populations but may overestimate or underestimate risk in certain individuals or subgroups. The calculator does not account for all possible risk factors such as family history of premature heart disease, inflammatory conditions, or certain ethnicities not represented in the original studies. Your healthcare provider can help interpret your results in the context of your complete health profile.
